INTAKE

If you are interested in having your child evaluated, please review the following intake forms to begin the process. We ask that you read our introductory letter, download and fill out the intake form, and fax or email it to the office for review (see Contact Us for our phone and fax numbers). Call our secretary to review the fee schedule with you. Our staff reviews each intake to make sure we are the right practice to evaluate your child.

After your intake is received, our secretary will call you about scheduling an appointment. If your insurance provider does not cover the visit and you are unable to afford the fee, please ask us about a payment plan.

REEVALUATIONS

If you are returning for a reevaluation, please call or email the office for an appointment, and download the Reevaluation Update Form. Fill it out and sign it, and return to the office with any supplementary reports, including progress notes from therapists, medical reports, or an updated IEP. If you would like us to provide you with a report for a specific meeting date, please call at least 3 months ahead to insure that you get an appointment at least a month before your meeting.
